Where Can I Print-off A Liable Power Of Attorney Form To Grant Guardianship Over My Child To A Close Friend?

Okay, so I have a close friend that lives in Oregon. I live in Arkansas. I am busy with work quite frequently, so I do not have time to make it to the courthouse. Is there anywhere I can print off a legal and liable form to grant this person guardianship but not full custody (aka, Power of Attorney) over my child?
