Okay, so I have a close friend that lives in Oregon. I live in Arkansas. I am busy with work quite frequently, so I do not have time to make it to the courthouse. Is there anywhere I can print off a legal and liable form to grant this person guardianship but not full custody (aka, Power of Attorney) over my child?

Basically, it says you can only apply on behalf of another adult [eighteen plus years of age] if: they are disabled, if you hold power of attorney, or if they are deceased. Well, I am not deceased.
